Default Kidney Failure??????

My grandpa has Alport's Syndrome. Yesterday his levels of creatine and potassium in his blood were high. I think he is supposed to go get checked out at the hospital today. How do they decide if you need a kidney transplant or dialisis? How long does it take for them to decide this?? Thanks!
